Nurse Aide- Hospice - Military veterans preferred

How You'll Help Transform Healthcare:

Provides personal care and comfort measures to patients of all ages in their homes as directed by the registered nurse.
 

• Assists with or provides basic personal care and hygiene to patients of all ages assigned and documents such care as well as significant findings;
• Provides specific aspects of care under the direction of the RN. Masters use of the laptop, demonstrating ability to review the care plan, and record the visit note. Understands when/how to use a paper note during downtime. Masters use of the mobile phone as applicable.
• Reports concerns or changes to the RN Case Manager/Clinical Team Leader;
• Assists patients of all ages promoting functional ability within the home environment;
• Assists patients of all ages in maintaining a comfortable home environment;
• Follows the established plan of care at all times;
• Participates in continuing education and other meetings;
• Demonstrates professional behavior and enthusiasm in daily activities;
• Responsible for abiding by all agency policies and procedures and;
• Performs other tasks as assigned; This may include cross-training to Hospice aide coverage.

What We Require:

Education: Demonstrate ability to read, write and communicate effectively. Satisfactory completion of Geriatric Nursing Assistant, Certified Nursing Assistant or Home Health Aide course that meets State, Federal, and CMS requirements for Home Health Aide training or is deemed competent by the home health agency as required by CMS regulations.

Experience: One (1) year experience in a healthcare setting preferred. Must have worked as an aide (for compensation) within the 24 months prior to hire or have completed another program as defined by the CMS Home Health Conditions of Participation.

Licensure, certification, and/or registration: Certified as a nurse's aide and registered by the Virginia Department of Health Professions. Must have license available to show for verification and primary source verification will also be conducted. Must have valid driver's license. Successfully completes HHA Competence Program as established by home health agency. Annually will complete by return demonstration all of the appropriate skills on the Home Health Aide competency checklist, and acquires twelve (12) hours in-service per year and receive a yearly evaluation of “meets standards” or better. AHA BLS-HCP must be secured by end of supervised orientation and maintained.

Other Minimum Qualifications: Must have reliable transportation. Clear criminal record check.
