We are seeking a full-time, remote Care Coordination Team Lead. The Care Coordination Team Lead is responsible for Home Health activities and assisting the Director of Post Acute Care and Home Health on team oversight. Responsible for performing a variety of duties including ensuring calls are appropriately answered and handled in a timely manner, proper document management, intake referrals, completion of letters of authorization, price negotiations and staffing home health and IV therapy referrals. Positively support the organization’s Mission, Vision, and Values. Work closely with referral sources, injured workers, and Home Health Coordinators to coordinate the smooth acquisition, workflow, and delivery of appropriate Home Health services. Provide excellent leadership, guidance, and assistance to the Home Health Team including training, coaching, and retention. LPN preferred.

The schedule for this position is Monday through Friday, 9a to 5:30p EST with some flexibility.

Duties and Responsibilities:

  • Provide appropriate, timely staffing for all home health referrals. Address all questions and concerns from adjusters, case managers, providers, and injured workers in a professional, timely manner.
  • Demonstrate excellent customer service and respect for customers, co-workers, and management, ensuring excellent product and service delivery and customer satisfaction for injured workers.
  • Maintain a high level of ethical and professional standards.
  • Demonstrate excellent leadership and applicable resources for the Home Health Team to promote a healthy work environment to include effective training, and customer service skills.
  • Communicate effectively with Home Health team members, management, internal and external customers, ensuring outstanding concerns within the Home Health Department are effectively addressed, e.g., staffing issues, follow up issues, cost-related issues, and client related concerns.
  • Evaluate and assess in network providers to determine reliability, professionalism, responsiveness, and outcomes.
  • Assist with on-call duties on assigned weekends, for which compensation will be provided.

Team Lead Responsibilities:

  • Mentor, train, and assist team members directly and indirectly, review their work and provide effective constructive feedback.
  • Ensure all team members understand, are trained in, and comply with Paradigm’s security requirements and policies.
  • Ensure all team members have the minimum level of IT system access required to effectively complete their Paradigm responsibilities.

Qualifications:

  • Education/Training: Minimum of a bachelor’s degree in a related field (e.g., Health Care Administration, Business, or Science). Commensurate experience may be considered in lieu of a completed bachelor’s degree. LPN preferred.
  • Experience: Work comp insurance, referrals, and authorizations experience preferred.
  • Skills: Excellent communication skills, verbal and written; Proficient in reading, grammar, mathematics, and medical terminology skills; Excellent computer skills (e.g., Outlook, Teams, to be trained on proprietary software); Excellent organizational and prioritization skills, with ability to multi-task.
  • Equipment/Machinery Used: Standard office equipment including phones, scanners, fax, laptop/PC, copiers, various computer programs, etc.
  • Able to cooperate and work cohesively with all co-workers, management, and external customers. Able to plan and complete job duties with minimal supervisory direction, utilize sound judgment, tact, diplomacy, and compassion in verbal and written communication.
  • Knowledgeable of injured workers’ rights and ensure an atmosphere which allows for the privacy, dignity, and well-being of each injured worker. Ensure confidentiality of all data, including injured worker’s demographics, employee, and operations data.
  • Able to complete requirements for in-service training, maintain acceptable attendance and appropriate dress code.
