Position Highlights:

What You’ll Do:

  • Execute incoming calls, assess client/patient needs, and meet patient needs as related to program scope.

  • Outreach to and assist patients with scheduling appointments within YVFWC.

  • Maintain patient medical records, update patient medical history and Social Determinants of Health (SDoH), current medical status including vital signs, medical treatments, current medications, and immunizations.

  • Collaborate with Care Management Nurse (RN) to ensure efficiency of schedules and continuity of care. Discuss patients scheduled for the day including pre-visit prep. Anticipate Care Management RN and program needs and manage Care Management RN and program requests.

  • Provide limited education and/or coaching for patients within scope. This includes discharge information and/or responding to follow-up questions. Instruct patients about medication and possible allergic reactions.

  • Respond to patients’ questions and concerns over the phone or electronically. If the question is outside of scope, direct inquiries to appropriate person.

  • Assist patients with completion of applications and forms related to necessary referrals. Ensure patient information is complete and entered in appropriate data system.

  • Provide follow-up contact with the patient regarding the service and referrals recommended. Encourage client participation and retention in various programs and classes.

  • Perform work to align with program goals and standards, reaching out to and responding to patients within designated timeframes.

  • Provide program administrative support such as data entry, client file management, report completion, and contract compliance.

  • Perform other duties as assigned.

  • Perform clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements.

Qualifications:

  • High School Diploma or General Education Diploma (GED)

  • One year of experience as a Certified Medical Assistant (MA) or Care Coordination

  • WA State CMA certification required

  • Bilingual (English/Spanish) required at a level 9

  • Ability to plan, coordinate and organize work projects.

  • Ability to provide professional written and verbal presentations.

  • Ability to prioritize work and handle a variety of tasks simultaneously, with frequent interruptions.

  • Ability to effectively interact with the public, patients, and clinic staff in a professional manner.

  • Ability to understand and apply program requirements and applicable local, state and federal guidance.

  • Knowledge of the social and health agencies in the area and services they provide.

  • Effective verbal, written and listening communication skills.

  • Basic proficiency with a variety of computer programs including Microsoft Office, Word and Excel.
