Description:

This position combines two important roles: Adult Case Management (50%) and Critical Time Intervention (CTI) Supervisor (50%).


A Case Manager coordinates a wide variety of therapeutic, educational, basic supports and integrative services to psychiatrically disabled individuals to ensure they receive the services and assistance required for successful maintenance in the least restrictive environment and for continuity of care. In addition, direct intervention in the community setting is provided to enhance goal attainment. This is an entry level professional position in community based psychosocial rehabilitation services for individuals with the most severe mental illness.


The CTI Supervisor position requires supervision of the CTI program and CTI Coach(es). The supervisor will be responsible for overseeing CTI coaches in tasks involving case management, care coordination, and transition planning for people transitioning from participating inpatient psychiatric hospitals. The supervisor will also attend all required CTI team meetings and trainings, as directed by the NH CTI Program directors, and be responsible for overseeing the success of this program at MFS. The supervisor must be able to step in when coaches are not available and will work directly with clients as needed. CTI clients may or may not be MFS clients.


Requirements:
  1. Bachelor's degree in social work, psychology, education, or related field, and 1 years' experience.
  2. DHHS approval is required if candidate has only an Associate degree or High School Diploma with appropriate work experience.
  3. An understanding of best practices within the MFS agency, including documentation and care provided.
  4. Demonstrated communication, leadership, program management, and training skills.
  5. A good driving record, reliable transportation, and the ability to transport clients when necessary to achieve treatment goals.
  6. Demonstrated ability to remain calm under pressure while making important clinical decisions and to develop and implement agency policies and procedures, and being a good role model.
  7. Shows a willingness to learn and keep a positive attitude.
  8. Good written and verbal communication skills.
