JOB RESPONSIBILITIES:
Serves as an educational resource for patients undergoing ostomy surgery when requested by the care team.
• Assesses patient/family perioperatively to devise a care plan for the promotion of physical, psychological, and social adaptation.
• Per physician order is responsible for preoperative selection of stoma site based on patient's anatomy, activity level, and lifestyle.
• Educates and counsels the patient and family perioperatively.
• Initiates appropriate discharge referrals based on evaluation of patient's physical, psychological and self-care status.
• Collaborates with staff to assist in educating and counseling patient and family.
Provides assistance to staff regarding management of patients with established ostomies.
• Assesses patient regarding self-care status and routines, knowledge level regarding ostomy diversion, and problems related to ostomy management or rehabilitation.
• Develops care plans to address needs relative to ostomy.
• Assists patient and/or staff with problem-solving as indicated.
Provides assistance to staff in management of patients with draining wounds and fistulas.
• Assesses patient to determine type and volume of output, perifistular skin status, abdominal contours, and activity level.
• Devises a plan of care for containment of drainage and odor and protection of perifistular skin.
• Educates staff/caregivers regarding wound management and helps as indicated.
Provides assistance to staff in the management of patients with potential or actual alteration inskin integrity. Assesses patients to determine type and volume of output, perifistular skin status,
abdominal contours, and activity level.
• Develops comprehensive wound management recommendation and assists staff in wound care when indicated.
• Evaluates patient response to treatment and modifies treatment plan as indicated.
• Educates patient and staff/caregivers regarding wound management plan.
• Provides guidance to staff in implementation of protocols to control or eliminate etiologic (risk) factors, including selection of appropriate support surface.
Assists staff to maintain current knowledge and competence in the areas of wound and ostomy care.
• Provides formal in-services as needed to ensure current knowledge and competence.
• Develops and revises policies, procedures, and guidelines related to wound and ostomy care.
Serves as a content expert for nursing staff.
• Analyzes stock items and recommends appropriate additions and deletions to assure quality and
cost effectiveness of products used for wound, ostomy, and continence care.
• Conducts product evaluations when indicated and submits reports and recommendations based
on results.
Participates in organization-wide committees and projects as requested.
Performs other related duties as assigned.

EDUCATION, EXPERIENCE AND TRAINING: Current Indian RN license required. BSN required. Certification required in Wound and Ostomy Care or completion of Certification within 12 months of employment. Minimum of one year experience as an RN.
