Physical Requirements
The physical requirements for this position include regularly sitting, standing, walking, with some pushing, pulling, and/or lifting objects weighing up to 10lbs and occasional pushing, pulling, and/or lifting objects up to 50lbs.
Shift
Monday – Friday, Day Shift; primarily 8:00 am – 5:00 pm (40 hrs/wk); other hours/schedule as needed.
Job Summary
The Nurse Coordinator will work with the Pulmonology – Lung Disease healthcare team, staff, and other providers to ensure excellent transitions of care within the healthcare setting.

This position focuses specifically on supporting patients with Interstitial Lung Disease (ILD) who are treated with complex medical regimens, including immunosuppressive agents requiring intensive monitoring. The Nurse Coordinator will help ensure that all relevant testing is completed, results are reviewed promptly, and any necessary changes to the care plan are effectively implemented.

This role encompasses both clinical and administrative responsibilities, including coordinating patient care, developing clinical workflows, and providing education to patients and staff. The nurse collaborates closely with a multidisciplinary team and uses clinical outcomes data to drive innovative, cost-effective improvements in care delivery. The Nurse Coordinator is accountable to patients and families for care processes and outcomes across each episode of care. They are also responsible to their peers for contributing to shared nursing practice and to the nurse/clinic manager for overall performance and outcomes.

The individual in this role will demonstrate competence in relevant clinical skills and knowledge and actively participate in the governance and improvement initiatives of the practice setting.
