Position Title:PPW SUD Recovery Support Navigation Specialist - Crowne

Location: Owensboro, KY

Description:

The PPW SUD Recovery Support Navigation Specialist provides peer-based support services to individuals with substance use disorders and/or co-occurring mental illness. This role involves guiding individuals through available resources while providing peer support, offering encouragement, sharing lived experiences, and supporting individuals through their connection to the recovery journey. The Peer Support Navigation Specialist works collaboratively to assist clients, their families, connect them with essential information and resources, fostering strong connections that enhance recovery outcomes. Facilitates scheduling of appointments, documentations and communicating with internal and external stakeholders for service navigation and support.


Essential Functions:

  • Peer Support: Provide one-on-one peer support to individuals with SUD, sharing personal experiences of recovery to build trust and encourage hope. Facilitates individual and group support in connecting to appointments, meetings, workshops, and recovery-oriented activities.
  • Resource Navigation: Assist clients in identifying and connecting with person-centered service needs, develop and facilitate scheduling for their individualized treatment plans, and collaborate with the treatment team to enhance recovery outcomes. In conjunction with the treatment, assist clients in identifying and accessing community resources, including healthcare, housing, employment, and other social services.
  • Advocacy: Advocate for the needs and rights of clients within various systems, including healthcare, social services, and legal systems. Educate clients about their rights and self-advocacy strategies.
  • Collaboration: Work closely with multi-disciplinary professional staff to coordinate care and ensure a holistic approach to recovery. Participate in case consultations, treatment planning, and team meetings.
  • Education and Outreach: Provide education to clients, families, and community members about resources on substance use disorders and recovery. Collaborate in outreach activities to engage individuals and families in services and raise awareness about SUD resources in their community.
  • Documentation and Reporting: Maintain accurate and timely documentation of client interactions, attendance, scheduling, progress, and outcomes. Distributes correspondence,
  • monthly calendars, special event updates, and communications to clients, as well as internal and external community stakeholders. Maintain and facilitate reporting on program and activity client engagement to supervisors and funding agencies as required.
  • Protect Confidentiality and Client Rights: Observe all rules of confidentiality relating to clinical information and treatment, both internally and when dealing with external agencies and/or individuals. Be responsible for understanding client rights and policy and procedures. Participate in staff training and development. Always maintain professional standards and observe guidelines established within the Code of Ethics and Conduct.
  • Training and Certification: Attend required Adult Peer Support Professional certification training within 6 months of employment and remain in compliance of Kentucky regulation 908 KAR 2:220 for Adult Peer Support regarding continuing education, and supervision.


Qualifications:

Education and Experience:

  • Education: High School Diploma or GED required
  • Age Requirement: Must be 21 years of age
  • Experience: Past or present consumer of mental health or substance abuse services with at least one year of sufficient progression toward recovery. Awareness of the importance of recovery in living with a mental illness or substance use disorder. Experience working directly with people in a service-oriented field preferred.

Knowledge and Skills:

  • Ability to effectively engage with diverse individuals.
  • Possess a basic understanding of mental health and substance abuse issues and their treatments.
  • Exhibit personal qualities such as patience, tolerance, compassion, maturity, understanding and empathy.
  • Knowledge of a variety of computer software applications in word processing spreadsheets, databases, and presentation software (i.e. MSWord, Excel, Access, PowerPoint).
