Job Overview

The Transition of Care Coordinator II provides support to the Transition of Care Program (TOC) and assists with case and care management. The Coordinator acts as a liaison for case managers to assist with the gathering of pertinent healthcare information, scheduling appointments, and follow-up on specialty/ancillary services. The Coordinator will generate/manage the daily census, post-discharge calls, and facilitate transitions of care. The Coordinator is responsible for processing referrals and serves as an associate and resource to patients, providers, staff, and external customers regarding policies, benefits, and care coordination. Participates in and provides report data as required. Lastly, the Coordinator is responsible for visiting patients at the bedside when needed to ensure smooth transitions for patients as they move to different levels of care, including enrollment in post-acute transitions of care programs.

Minimum Requirements

  • High School Diploma or equivalent required.

  • Medical Assistant Certification preferred.

  • Prior experience working in a clinic/health care call center.

  • Minimum 2 years of experience working in a health care environment; knowledge of prior authorization and case management regulations governing Medi-Cal, Commercial, Medicare, CCS, and other government and commercial programs.

  • Experience in a managed healthcare environment, preferably IPA, HMO, or Health Plan, is preferred.

  • Experience working with an ethnically diverse population is preferred.

  • Bilingual English/Spanish is strongly preferred.

  • A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Compensation

$25.00 - $29.32 hourly
