Overview

As a key coordinator of patient placement, you ensure that each individual receives timely and appropriate care within our facilities. Your efforts help streamline ED admissions, direct admissions, and hospital-to-hospital transfers—making these transitions as smooth as possible. Responsibilities

  • Coordinate patient placement for all incoming patients across all Legacy facilities in a 24/7 operational setting

  • Collaborate with hospital charge nurses and Nursing Supervisors to ensure accurate placement of:

    • Emergency Department (ED) admits

    • Direct admits

    • Hospital-to-hospital transfers

  • Manage all aspects of patient transfers from outside facilities into the Legacy system

  • Facilitate physician-to-physician handoffs to maintain continuity of care during transfers

  • Act as a liaison for internal and external physicians requesting consults, admission support, or patient information

  • Monitor and track:

    • Bed assignments and availability

    • Unit staffing capacity

    • Patient discharges

  • Ensure accuracy of the nightly hospital census report



Qualifications

Education:

  • High School diploma or equivalent required.
  • A minimum of two years college education including satisfactory completion of college level Health Records coursework preferred.

Experience:

  • A minimum of one year of healthcare experience or equivalent education in at least one of the following areas required: Admitting, Medical Records/Health Information or applicable clerical support experience.
  • Six months customer service experience required.
  • Previous registrar and third-party payor experience preferred.
  • An understanding of health plan and benefit structures preferred.

Skills:

  • Demonstrated ability to communicate clearly in the English language.
  • Ability to withstand varying job pressures and organize and prioritize related tasks.
  • Ability to work efficiently with minimal supervision, exercising independent judgment within stated guidelines.
  • Demonstrated effective interpersonal skills which promote cooperation and teamwork.
  • Excellent public relations skills and demonstrated ability to communicate in a calm, succinct, businesslike manner.
  • Ability to identify alternative means of communication as needed.
  • Net typing of 40 WPM and PC-based computer skills.
  • Demonstrated knowledge of medical terminology.
  • Familiarity with 10 key.
