Emergency Medicine Clinical Pharmacist

Position Purpose: The Pharmacist is responsible for educating, assisting, and interacting with patients in an outpatient clinic setting with regards to health promotion and wellness.

Duties and Responsibilities:

  • Works collaboratively with multi-disciplinary teams to optimize drug therapy.
  • Provide direct patient care, including treatments assessment and medication management activities.
  • Clinically evaluate all patients in a specified area daily.
  • Ensure documentation is placed in the medical record regarding pharmacy recommendations, plans, intervention and medication reconciliation in medical records.
  • Review patient’s drug regimen for an appropriate indication, dosing, allergy verification, potential drug interactions, lab monitoring, duplication of therapy, and medical documentation.
  • Provide accurate and timely drug information to physicians, nurses and other health professionals.
  • Respond to level 1 trauma, code blue massive blood transfusion emergencies.
  • Provide direct patient care and medication therapy monitoring, including antibiotic stewardship, pharmacokinetic drug dosing, renal and hepatic dosing and patient education.
  • Develop, implement, and monitor clinical protocols.
  • Provide consultative services and clinical expertise to evaluate and improve medication usage and therapeutic outcomes.
  • Process orders and prescriptions to provide medication and pharmaceutical supplies ordered by physicians and authorized medical practitioners.
  • Facilitate discharge planning and smooth transition to alternate sites for care from a pharmacy perspective.
  • Provide cost-effective management of drug utilization within the clinic.
  • Monitor polypharmacy and high dose pharmaceutical dosing to prevent adverse effects.
  • Provide drug information to patients and their families.
  • Demonstrate professional communication with patients, family members, professional and support personnel involved in the delivery of patient care.


Qualifications

Requirements:

  • Completion of ASHP accredited PGY1 Pharmacy Practice Residency AND completion of ASHP-accredited PGY2 in Emergency Medicine Pharmacy or Critical Care Pharmacy OR
  • Completion of ASHP accredited PGY1 Pharmacy Practice Residency AND 3 years of practical clinical practice experience in Emergency Medicine with BPS board certification.
  • American Heart Association Basic Life Support (BLS) certification.
  • American Heart Association Advanced Cardiovascular Life Support (ACLS) certification.
  • Meets standards of an ASHP-Qualified Residency Preceptor within one year of employment.
  • Bachelor’s degree in pharmacy. Must be obtained from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
  • Minimum of one year of pharmacy experience within the last three years
