JOB SUMMARY

The Family Navigator is a key member of the North Hartford Ascend Pipeline (Ascend) team. This role supports Ascend: a place-based, cross-sector initiative that integrates schools and community and social services and supports to help children and families reach their full potential. The Family Navigators will advocate, mentor, engage, empower, and support families to take the lead in their own Plan of Care and helps connect families to needed resources within their own communities. Services are in-home and/or within the community. Family Navigators will guide families to services and assistance from the community resources through services. Ascend is funded by the U.S. Department of Education Promise Neighborhoods program and sponsored by Connecticut Children’s.

KEY RESPONSIBILITIES

  • Recruitment and engagement of families into the Ascend pipeline of services.
  • Identifying and utilizing community resources and supports to meet the needs of the family.
  • Referring and connecting families to support programs and services so families reach their optimal functioning.
  • Empowering families through education and support to enable them to take the lead role in planning for and responding to their individual family’s needs.
  • Will role model and provide feedback to cultivate nurturing relationships with families and develop connections for families to access resources in the community.
  • Works as a team member with other program staff to respond to family and programmatic needs.
  • Develops and maintains collaborative relationships with other community agencies and organizations in consultation with supervisor, to provide comprehensive and effective treatment services for families (i.e., schools, courts, hospitals, etc.).
  • Accurate and timely completion of documentation and reports, and any other documentation and reports required by agency and program policies and funding sources.
  • Partners with supervisor to identify areas and develop plans for professional growth.
  • Contributes to agency visibility and development of community linkages by representing the Ascend and Village in community groups or committees, as assigned by direct supervisor.

Experience and Skills

EDUCATION

  • High school diploma with experience in family skills and community services.

EXPERIENCE

  • Bilingual - Spanish preferred
  • Must have a current valid state issued Driver License and own a vehicle for home visits, community events, etc.
  • Strong communication skills both verbal and written.
  • Solid general computer skills such as typing and navigating websites is required.
  • Physically able to perform the essential functions of the position, with or without reasonable accommodations
