Service Coordinator - $53,000 - Hybrid Position
$23.07 - 25.74 per hour, based on experience + benefits, + $2000 Signing Bonus (non-exempt)
Hybrid remote opportunity - work frm home but required to travel within St. Louis County, MO
 
SUMMARY: Our client has provided exceptional opportunities for individuals with intellectual and developmental disabilities since 1955. They provide programs and services to assist individuals with disabilities, low-income, at-risk children, and their families to meet their goals and experience their best lives while living, working, playing, and becoming a part of their communities. The service coordinator helps people meet their personal needs and goals in obtaining the greatest degree of independence and inclusion possible in everyday community life. They help people with developmental disabilities and their families identify and obtain needed services and support. They also advocate for, monitor, and evaluate services along with the individuals, their families, or guardians.
  • Planning support: There are two components- Working with the individual and their support team to identify the needs of the person being supported and creating an action plan that will support the person in meeting those needs
  • Linking Resources: Matching the unique support needs of individuals and families (identified in the plan) with resources in the community. Linking may involve researching, developing making referrals to collaborating agencies including federal, state, county, and community programs to achieve necessary supports
  • Quality Enhancement: Monitoring and assessing the quality, timeliness and effectiveness of services and support received through an ongoing review process. Monitoring occurs via phone or email contact and in-person meetings. In-person meetings are completed Monthly, Quarterly, and Annually. These meetings can occur in various settings including but not limited to homes, day programs, community settings, office settings, group homes, and residential settings, etc. If there are findings from the monitoring reporting processes will be followed
  • Documentation: Complete type written case note documentation for all activities completed for each individual
  • Building Relationships: Establishing strong, trusting relationships with the person being supported and their natural and/or paid support team. This is an important element of quality service coordination. Mandated Reporter: Ability to identify potential abuse and neglect situations, and follow protocol according to all applicable regulatory requirements
  • Networking: Developing community relationships and support systems that benefit the lives of individuals with developmental disabilities and their families
  • Knowledge Enrichment: Completing all training as assigned. In addition, continuing one’s education and professional growth. Completes training as stated by policy
REQUIRED EDUCATION
  • Bachelor’s degree from an accredited college or university or Registered Nurse license 
 REQUIRED MISCELLANEOUS QUALIFICATIONS
  • Valid Class F driver’s license
  • Reliable transportation, an acceptable driving record and personal insurance
  • Must have access to reliable internet for telecommuting employees
  • Must complete a TB Assessment and/or requirements
PREFERRED EXPERIENCE
  • Working with individuals with intellectual and/or developmental disabilities in a community-based setting as a Registered Nurse, or in social work, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, or a related area
  • Working with various case management and documentation applications such as but not limited to Setworks, Kronos, Therap, etc. 
  • As a member of a multi-disciplinary team in the areas assessing, developing, or implementing individual person-centered plans, especially for persons with intellectual/developmental disabilities
  • Utilizing the Microsoft Office Suite
