Job Description:CVHCare is looking for a qualified Physical Therapy Assistant with a minimum of 1 year professional experience. Home health experience is preferred. Assigned county Territory is Area #10 (San Mateo, Brisbane, Burlingame, Daly City, Redwood City, Hillsborough, Millbrae, Colma, South San Francisco, Pacifica, Palo Alto, Belmont, San Bruno, Half Moon Bay, San Carlos, Menlo Park, El Granada) AND Area #9 (San Francisco) TWO Options - Guaranteed Hours or Non Guaranteed hours. If Guarantee, the following shall apply: Productivity: At least 30 points per week (points described below). 36 hours Guarantee. Traveler must complete 30 points in the week. Must be willing to travel outside the area or work weekends if required to maintain weekly visit expectations. If Traveler refuses patients, guarantee does not apply. Guarantee does not apply for 1st and last week of assignment, or for weeks in which there are holidays, or for weeks in which the traveler has time off.If Non-Guarantee, the following shall apply: Weekend coverage may be required. To secure a full caseload, Traveler may be required to go outside the service area occasionally. Can refuse patients. No guaranteed hours. Traveler must be willing to see Covid+ and Covid Pui patients. Payment made on per visit basis. See below for visit types and how they convert to points and how they convert to hours.This is how we will calculate how many hours are approved per day.Routine Visit = 1 point = 1.33 hours.Although Travelers are paid hourly by their agency, hours are approved by CVHCare on a per visit bases. Note that Rate is all inclusive and covers drive/travel time, visit time, documentation time, & time spent in making phone calls to patients & doctors.Credentials: Current CA PTA License, BLS, Drivers License, Auto ins, Auto Reg, TB (annual), MMR, Varicella, HepB, TDap (10 years), Flu (annual), Drug screen, Physical.Your Agencys contract to your traveler MUST mirror the confirmation page. If your agency does not comply with this standard, your agency will be at risk of being removed from this clients vendor set. No exceptions! Start Date Must be Identical. End Date Must be Identical. ***Please Note - All Requested Time Off (RTO) must be disclosed at the time of submission. If your agency or the traveler does not include all RTO in the submission form and adds it post confirmation, your agency will be at risk of being removed from this clients vendor set. No exceptions! Agency employee must work at least 3 scheduled shifts after completion of orientation for Agency to be paid for orientation hours. Orientation is paid at *** less per hour than the all-inclusive agreed upon hourly rate*** Overtime is paid at *** more per hour than the all-inclusive agreed upon hourly rate*** Overtime is paid after 40 hours (30 RVEs) per week. All approved worked hours will be calculated based on the below Point Value/Hourly conversion table below. Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/up, Re-Eval, Eval Discharge, Oasis Discharge, Oasis F/up = 1 point = 1.33 hours. OASIS SoC and OASIS ReCertification and OASIS RoC = 1.5 point = 2 hours.Note that Rate is all inclusive and covers drive/travel time, visit time, documentation time, & time spent in making phone calls to patients & doctors. Agency employee will be issued a Smartphone and medical devices (stethoscope, pulse ox, gait belt, blood pressure cuff, etc). Any lost, stolen or damaged equipment will be the sole responsibility of the Agency to fully reimburse the Facility. Facility data plan is optimal for full time field clinicians (documentation, texting, calls, hotspot). Data usage will be kept to a strict minimum, reasonable and necessary for business. Facility has established a monthly max limit of 3 GB per user.
