Job Description

  • Work remotely with the use of company issued computer and cell phone.
  • Employee will come on-site weekly for mail pick-up, letter and records drop off and for any other department business.
  • Perform inpatient reviews at each facility daily where patients of HVVMG are hospitalized as assigned daily. (This includes any OOA facilities deemed necessary for inpatient review as determined by the CM Director.)
  • Perform inpatient daily reviews at the SNF facilities where HVVMG patients are assigned. (This includes any OOA facilities deemed necessary for inpatient review as determined by the CM Director.)
  • New Admissions to SNF to receive in-house visit for local SNFs within 48 hours. Weekly visits to all local SNF patients for updating plan of care and Discharge process.
  • Review medical record of each patient for SI, IS criteria. Acute Case Daily, SNF Cases daily- utilizing required criteria guidelines provided by HVVMG.
  • Communicate with attending physician regarding any utilization of facility service issues that are noted.
  • Identify and initiate discharge planning needs. Acute Case Daily, SNF Cases daily- utilizing required criteria guidelines provided by HVVMG.
  • Assist attending physician to identify continuum of care needs. Acute Case Daily, SNF Cases daily- utilizing required criteria guidelines provided by HVVMG.
  • Facilitate discharge plans. Acute Case Daily, SNF Cases daily- utilizing required criteria guidelines provided by HVVMG.
  • Report inpatient status daily to director. Acute Case Daily, SNF Cases daily- utilizing required criteria guidelines provided by HVVMG.
  • Attend SNF Daily Utilization Review meeting as assigned.
  • Maintain Utilization Review Documentation per CM Department policy. Acute Case Daily, SNF Cases daily- utilizing required criteria guidelines provided by HVVMG.
  • Communicate Daily with CM Team Coordination Team regarding all discharged patients per CM Department Policy.
  • Assist with patient issues as assigned by Supervisor.
  • Perform other duties as assigned.
The pay range for this position at commencement of employment is expected to be between $28.00 and $36.00/hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience. The total compensation package for this position may also include other elements, including a sign-on bonus, restricted stock units, and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered. Details of participation in these benefit plans will be provided if an employee receives an offer of employment.

If hired, employee will be in an "at-will position" and the Company reserves the right to modify base salary (as well as any other discretionary payment or compensation program) at any time, including for reasons related to individual performance, Company or individual department/team performance, and market factors.

Requirements
  • High school graduate or equivalent.
  • Graduate from an accredited Licensed Vocational Nursing program.
  • Current California LVN or RN license.
  • Current CPR certification.
  • Minimum two years of clinical nursing experience.
  • Excellent written and verbal communication skills.
  • Ability to handle and prioritize tasks in order to meet all given deadlines and productivity goals.
  • Ability to responsibly handle matters of a confidential nature.
  • Ability to work in a multi-task, high-stress environment.
