Performs duties with focus on consumer-centeredservices, consumer choices, discretion and attention to detail in accordance with the agency's missionSome Duties and Responsibilities*Manages a case load of 7-10 individuals to complete quarterly reports, plan updates, falls riskplans/assessments, annual paperwork, support instructions, and assessment updates as needed.*Provides support at annual meetings, client care specific meetings, and supports the completion of rootcause analysis for individuals on assigned caseloads.*Will participate in On Call responsibilities for assigned Director Team, and respond to individual careneeds and or critical staffing issues as needed.*Professionally communicates and advocates for clients in accordance with agency's code of ethics andall applicable DHS and Human Rights compliance.Performs other related duties as assigned.Education and Experience Requirements*Bachelor's Degree in Human Services or field related to community services*One or more years of directly related experience or any combination of training and experience whichprovides the required knowledge, skills, and abilities*Valid Virginia Driver's license and acceptable Department of Motor Vehicles Driving record anddependable transportation*Able to move safely within the home environment, which may include stairs and multiple stories*Able to lift up to 50 pounds to assist individuals who have physical challenges with positioning,transferring or ambulating as neededDTG ADVANTAGES* Work with one of the nation's largest behavioral health referral agencies* Flexible schedule as a Contractor.
