Overview

We are assisting our client in their direct hire search for a Case Manager. This is a FT, 40 hours, non-exempt, hybrid position located in their San Mateo office. The program is a comprehensive, full-service partnership (FSP) for 45 high risk children/youth living in San Mateo County. It is designed to help children and youth achieve independence, stability, and wellness within the context of their culture, community, and family/caregiver. The Case Manager is a member of their multidisciplinary team whose role is to serve parents, caregivers, and non-professional supportive adults of our youth (ages 6-21). This individual helps coordinate the treatment of youth and families among internal and external (which may include schools, mental health, physical health/medical, juvenile justice and/or legal, child welfare systems) providers.

Responsibilities

  • Provide counseling and behavioral interventions, guidance, skills-building, and support in accordance with each client's individualized treatment plan and the program's treatment philosophy and model.
  • Provide services to 6 - 21-year old's and their families in the home, at school and/or in community settings, as required by the treatment plan.
  • Engage family members and caregivers as full partners in service planning, delivery, and evaluation.
  • While under the Case Manager's care, always maintain supervision of clients to ensure their safety and well-being.
  • Ensure facilities, living quarters, office spaces, equipment, and furniture to be used in providing services are in good working order and professionally maintained at all times.
  • Engage in crisis interventions and de-escalation, including the application of conflict resolution and collaborative problem-solving techniques.
  • Implement clinical policies and procedures and actively enhance communication, coordination, and integration of care with other service areas and with external providers.
  • Contribute to organization-wide quality management and improvement efforts by consistently providing the highest quality professional interventions, fulfilling all documentation requirements, and proactively seeking ways to enhance the quality of services and surpass minimum required standards.
Preferred Requirements
  • Bachelor's degree required, equivalent education and relevant experience may be considered in lieu of a bachelor's degree.
  • Demonstrated ability to work with families in a wide variety of cultural contexts and community settings.
  • Must have a valid California driver's license and proof of insurance.
  • Understands the skills needed to partner with treatment team members in the creation of holistic treatment and individualized care plans.
  • Case management, mentoring, coaching, and facilitation skills.
  • Mentoring, advocacy, and empowerment skills.
  • Develop, maintain, and strengthen relationships with supportive family networks, services, and resources.
  • Strong verbal and written communication skills.

Salary

This position offers an hourly rate of pay of $24.75 per hour to $27.23 based on experience and bilingual skills. In addition, they offer an excellent comprehensive benefits package.
