A full-time Young Adult Case Manager is assigned as many as 15 participants. Responsibilities of the Young Adult Case Manager are to facilitate enrollment, manage continued eligibility, assist with housing, assisting with life skills, and case management needs in a timely manner. Young Adult Case Manager will build rapport with and respect participant's role. Ensures eligibility documents are maintained and document all critical incidents in a timely manner, and attend court hearings with participants as needed. Complete case documentation for all visits, calls, texts, emails, etc. within timely manner. Maintain all case documentation in SACWIS. Completion of assessments, case plans, reviews, complete required trainings, etc. This position will primarily work out of our Canton office, but at times may commute to the North Olmsted office.

The qualified applicant shall have a post-secondary program in social work, human services, behavioral health or related field and have at least two years' experience working with families and/or children in the child welfare system or possess a bachelor's degree in social work, human services, behavioral health, or related field. Must have a demonstrated ability to work well with many different types of people, possess a valid driver's license, have a good driving record, carry automotive liability insurance and have a car which is safely maintained. This person must meet the background screening as defined in OAC code 5101:2-5-09. Demonstrates sensitivity and consideration for the cultural differences among children and families served by Pathway.
