Description

SUMMARY OF POSITION:

Springs Rescue Mission considers every position one of ministry and a vital and valued part of our team. To that end, SRM creates Whole-Person programs that encourage steps toward a better future of self-sustainment in Health, Work, and Housing. Case Managers are integral in promoting progress by persistently advocating for the Hope Program. Clients may choose to follow other avenues, but Springs Rescue Mission believes the Hope Program is transformational. For that reason, Case Managers will always direct clients toward the Hope Program processes to ensure that clients receive the best possible program. In addition, this position will collaborate with all program departments to provide complete wrap-around services for each client. Incumbent will also track all needed information in the SRM client tracking software to ensure the program participants are progressing through their program toward self-sufficiency in health, work, and housing.

DUTIES & RESPONSIBILITIES:

  • Follows Hope Program Case Management policies and procedures.
  • Ensure all clients are following Hope Program guidelines.
  • Communicates with SRM program staff to provide complete wrap-around services for clients.
  • Always holds clients supportively accountable toward progress in Health, Work, and Housing through the Hope Program.
  • Tracks client progress through each phase to ensure clients meet phase timelines.
  • Maintains an acceptable caseload based on Hope Program guidelines
  • Attends wrap-around service meetings to discuss the progress of clients in the Hope Program
  • Captures relevant notes and updates to cases in a consistent, accurate, and timely manner ensuring that all clients are following the Hope Program guidelines.
  • Supports proper training and recognition of volunteers for their partnership with Springs Rescue Mission and engages them in meaningful work that leverages volunteers as a resource.
  • Additional duties may include driving, driving records will be pulled and submitted to insurance along with adherence to SRM driver safety policies.
  • Attends all required meetings and reviews SRM emails regularly
  • Duties and responsibilities may be added, deleted, or changed at any time at management's discretion, formally or informally, either verbally or in writing.
Requirements

EDUCATION/EXPERIENCE:
  • Minimum of bachelor's degree in related field or 5 years relevant work experience in similar environment preferred
  • Five years or more of experience working within a Christian recovery environment desired
SKILLS AND QUALIFICATIONS:
  • Maintains a personal relationship with Jesus Christ and is a constant witness. Christ-centered and compassionate attitude in dealing with people including the homeless and faithfully upholds and exemplifies SRM's Statement of Faith
  • Serve as an advocate for the Hope Program
  • Actively support our belief in the value of volunteers and interns to help accomplish the vision and mission of SRM and thus mobilize our community in the fight against addiction, poverty, and homelessness
  • Able to partner and collaborate effectively with other agencies, professionals and clinicians in social work and related disciplines
  • Basic knowledge of our community of Colorado Springs, other agencies and resources that are involved with homelessness and extremely low-income individuals
  • Experience working with clients undergoing trauma, addictions, physical and mental challenges, and homelessness
  • Capable of being a supportive, engaged, active, encouraging teammate and collaborator with other SRM staff members and teams
  • Experience with case management concepts
  • Some exposure to complex, multi-agency environments involving government agencies
  • Comfortable learning client management systems and software.
  • Organized, patient, focused and solution-oriented and capable of understanding and empathizing with clients representing their best interest
  • Able to hold clients accountable, using agreed upon decision matrix, to SRM and Hope program participation expectations
  • Ability to establish and maintain appropriate limits, expectations and boundaries for healthy self-care
  • Ability to maintain confidentiality with clients, partners, coworkers, and the community
WORKING CONDITIONS/PHYSICAL FACTORS:

Occasionally = 1%-33%; frequently = 34%-66%; continuously = 67%-100%.
  • Varying typical working conditions to include office, shelter environment, outdoors, and within community.
  • Occasionally: must be able to lift to 20 pounds, bend or stretch
  • Frequently: climbing stairs, sitting, walking, talking, manual dexterity, regular interactions with a low-income, disenfranchised population including persons that are mentally ill, addicted, and homeless
  • Continuously: adequate vision and hearing, good mental condition to withstand consistently medium to high levels of stress

Salary & benefits include the following: $22.60 - $24.00/hour , DOE.

* Full benefits package to include: medical benefits with SRM paying 85% of the medical premium, voluntary Dental, Vision, Allstate Accident Plan, SRM-paid Short-Term and Long-Term Disability, Life Insurance, and AD&D. Also offered are 8 paid holidays, 21 days of accruable PTO, paid "Soul Care Retreat" days (up to 4 depending upon availability), Pay on Demand, and a 403b retirement plan with SRM-match after one (1) year, 100% vested beginning DOH; (Benefits are subject to change based on reevaluation or benefit plan changes annually.)
