In collaboration with others, carry out an individualized, comprehensive, and patient centered Plan of Care in accordance with current applicable Federal, State and local standards, guidelines and regulations as may be directed by the Physical or Occupational Therapist, Rehabilitative Services Manager, and/or Director of Patient Care Services.Providence's Post-Acute Care facilities offer a sense of community and home to all our resident patients.
