A minimum of one letter must be from a supervisor. Recommendation letters must include the recommendation recipient's name, title, phone number, date of reference, address, and signature.Duties:Perform a full range of Physical Therapy Assistant services in support of patient referrals from the following specialties: Orthopedics, General Medicine and Surgery, Primary Care Clinic, Rheumatology, and other referrals approved by the Department Head/Division Officer, within the scope of this statement of work, on-site, using Government-furnished supplies, facilities, and equipment. Services provided under this contract shall be performed by all DoD, Navy, and local MTF guidelines and reporting requirements and by the American Physical Therapy Association (APTA) guiding principles.Start date: As soon as possible pending Federal Security Clearance process completion (typically 4-6 weeks after initiation)Schedule:Services shall be provided Monday through Friday and federal holidays between 0600 to 1800 hours. Shifts will generally be scheduled for an 8.5 to 9-hour period, including an uncompensated 30 to 60-minute lunch break. The HCW may be scheduled to provide services during the weekend to augment Government personnel. In addition, the HCW may provide services to patients between 0600 and 1800 on Saturdays and Sundays. Historical data indicates the number of Contractor weekend assignments is approximately four (4) instances during the period of performance of the task order, at a frequency of less than or equal to one weekend per month (historical data does not necessarily reflect future requirements). In no instance will the HCW be required to provide services over 80 hours per two-week period.Ability to commute:Transportation is required and expected for commuting to and from the work site.Experience:Possess a minimum of 12 months experience as a Physical Therapy Assistant within the preceding 36 months.
